ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Transition Support Specialists work directly with participants in classroom, community, and work-based learning environments, reinforcing program expectations, promoting independence, and supporting individualized goals. This role is essential to maintaining a high-quality, person-centered program experience.
- Participant Support & Engagement
· Support participants in developing life skills, career readiness, and independence.
· Reinforce daily routines, schedules, and program expectations.
· Promote participant choice, self-advocacy, and accountability.
· Support participants during community-based learning, work experiences, and skill-building activities.
· Encourage appropriate workplace behaviors, communication, and professionalism.
· Facilitate small-group and individual activities aligned with program curriculum.
· Support participants across multiple training environments, including classrooms and community sites.
· Assist with hands-on learning activities.
· Maintain appropriate staff-to-participant ratios (1:10).
· Follow all safety procedures and guidelines.
- Documentation & Communication
· Accurately document participant attendance, engagement, and progress as required.
· Communicate participant needs, successes, and concerns to the Program Operations
Lead.
· Maintain confidentiality and professional boundaries at all times.
· Participate in team meetings, trainings, and supervision sessions.
· Work collaboratively with other Transition Support Specialists and program staff.
· Support smooth transitions between program phases and services.
· Assist with special events, employer visits, and program activities as assigned.
· Provide input on participant progress and program improvements.

QUALIFICATION REQUIREMENTS:
Strong interpersonal and communication skills. Ability to support multiple participants while maintaining structure and consistency. Comfort working in both classroom and community environments. Reliable transportation for local travel. Ability to drive commercial van or bus.
Education and/or Experience:
Associate or bachelor’s degree preferred in human services, education, social work, psychology, or related field. At least one year of experience working with individuals with disabilities, youth, or adults in a support, education, or community-based setting.
Language Skills:
Ability to understand basic Agency and DHS regulations and basic written directions. Ability to document specific date. Must have the ability to work cooperatively and efficiently with clients and other staff, and practice good judgment when dealing with all people.
PHYSICAL DEMANDS:
Ability to stand, walk, and move throughout the day. Ability to lift up to 25 pounds as needed for program materials. Ability to assist participants during community activities. Willingness to travel locally to employment or training sites.

Pay: $14.00 - $16.00 per hour
