New London Nursing and Rehab is a skilled nursing and rehabilitation center committed to high-quality, resident-centered care and strong clinical outcomes. We are currently seeking an experienced and detail-oriented MDS Coordinator to join our team. This role is ideal for a clinician looking for a balanced schedule while continuing to play a key role in care coordination and compliance.
What You’ll Do:
Coordinate and complete accurate, timely MDS assessments in accordance with state and federal regulations
Manage the RAI process and support interdisciplinary care planning
Ensure documentation supports quality outcomes and reimbursement
Collaborate with nursing, therapy, and administrative teams
Assist with audits, surveys, and quality improvement initiatives
What We’re Looking For:
MUST have Active RN or LPN license in good standing
Prior MDS experience in a long-term care setting preferred
In-House Position
Strong knowledge of Medicare/Medicaid guidelines and RAI requirements
Excellent organizational skills and attention to detail
Ability to work independently and manage deadlines
