Overview
We are seeking dedicated Licensed Practical Nurses (LPNs) to join our healthcare team for 12-hour day shifts. Duties
- Deliver comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations to inform care plans.
- Administer medications via injections, IV infusions, and other routes while adhering to HIPAA regulations and safety protocols.
- Manage complex medical documentation using EMR systems such as Epic, Cerner, or Athenahealth to ensure accurate and timely recordkeeping.
- Provide specialized care for patients with dementia, Alzheimer's disease, or memory impairments within memory or behavioral health units.
- Assist with invasive procedures such as catheterization, tube feeding management, wound suturing, and spinal taps under physician supervision.
- Support acute pain management strategies and participate in case management activities for post-acute or hospice care patients.
- Collaborate with interdisciplinary teams to develop discharge planning strategies and coordinate home & community care services.
- Utilize physiology knowledge and diagnostic evaluation skills to monitor patient status in emergency room, ICU, or Level I trauma center settings.
- Conduct patient education on medication administration, medical terminology, infection control practices, and health coaching techniques.
- Ensure compliance with infection control standards and sterile processing protocols while maintaining a safe environment for patients and staff.
Experience
- Proven clinical experience in diverse healthcare settings such as hospitals (including ICU, PICU), nursing homes, assisted living facilities, or outpatient clinics.
- Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, or Athenahealth for documentation and patient record management.
- Critical care experience including ICU or Level I trauma center exposure is highly desirable.
- Experience working with individuals with disabilities or developmental delays in home care or community-based programs.
- Familiarity with ICD-9/10 coding systems for accurate medical documentation and billing processes.
- Knowledge of infection control procedures, sterile techniques (such as suturing), and advanced life support measures including ventilator management.
- Background in managing complex cases involving infectious disease care, pain management strategies, or hospice services is advantageous.
- Strong understanding of anatomy, physiology, pharmacology related to medication administration and tube feeding procedures.
