Overview
Position Summary We are seeking a compassionate, organized Case Manager and Group Facilitator to support individuals in our substance use treatment program. This role combines case management duties—assessing needs, coordinating services, and supporting discharge planning—with leading evidence-based group therapy sessions. The ideal candidate is skilled in engaging diverse populations, collaborating with multidisciplinary teams, and promoting recovery-oriented goals in a trauma-informed, patient-centered environment.
Key Responsibilities
o Case Management
o Conduct comprehensive psychosocial assessments to identify barriers to treatment (housing, employment, transportation, legal, family supports).
o Develop and implement individualized service plans with measurable goals in collaboration with clients.
o Coordinate and monitor linkage to internal services (therapy, psychiatry, MAT coordination) and external resources (primary care, housing, employment programs, benefits).
o Facilitate care coordination across disciplines; ensure timely access to services and continuity of care.
o Provide crisis intervention, safety planning, and risk assessment as needed; document all interactions thoroughly.
o Track progress, update treatment plans, and prepare discharge planning that supports ongoing recovery.
Group Facilitation
o Plan, facilitate, and evaluate evidence-based group sessions (e.g., relapse prevention, coping skills, 12-step inspired facilitation, psychoeducation) per program model.
o Create a supportive, inclusive group environment; manage group dynamics, maintain boundaries, and ensure member safety.
o Adapt group content to diverse literacy levels, cultures, and treatment stages; incorporate motivational interviewing techniques as appropriate.
o Maintain attendance records, session summaries, and progress notes for group participants.
Documentation and Compliance
o Maintain accurate, timely documentation in electronic health records; ensure confidentiality and compliance with HIPAA and program policies.
o Complete required forms for assessments, treatment plans, progress notes, and discharge summaries.
o Contribute to quality improvement initiatives and data collection as requested.
Collaboration and Communication
o Work closely with clinicians, medical staff, and program leadership to coordinate care.
o Communicate client progress, barriers, and needs with the treatment team; participate in case conferences and team meetings.
o Engage families and natural supports when appropriate and with client consent.
Outreach and Linkage
o Assist clients in connecting with community resources (housing, employment services, legal aid, education programs).
o Support referrals to ancillary services and track follow-through

Pay: $17.00 - $20.00 per hour
