Social Worker - Nursing Home

A beautiful skilled nursing facility located in The Bronx, NY is seeking a strong Social Worker with a background in long-term care. This role is responsible for supporting residents and families, coordinating care planning, and ensuring compliance with regulatory requirements.
Salary : $80k to $85K
Social Worker Job Description:
- Assist new residents and their family in the initial adjustment phase, as well as in the ongoing transition to institutional living
- Provide information to the resident, and/or family member, on residents' rights and responsibilities
Qualifications:
- Must have BSW, MSW, or LMSW
- Must have previous experience as a Social Worker
- Must have long-term care experience
- Must have strong organizational and case management abilities
- Strong written and verbal skills
- Excellent verbal and written communication skills
