Registered Nurse - Care Coordination
Pay: $87,000.00-$102,000.00
Schedule: Monday through Friday: 8:00am to 4:00pm
Seven Hills Foundation is seeking a Registered Nurse (RN) – Care Coordination to oversee and coordinate medical services for individuals in our school and residential programs.
This role combines clinical nursing, care coordination, and team leadership. You’ll advocate for the health and well-being of program participants while working closely with physicians, families, residential teams, and nursing staff to ensure individuals receive safe, effective, and person-centered care.

What You'll Do:
Assess and coordinate healthcare services for students and program participants.
Oversee medical care and nursing services across assigned programs.
Supervise, support, and train nursing staff and provide oversight of Medical Technicians.
Coordinate with physicians and other healthcare providers regarding participant needs, treatment, and follow-up care.
Monitor and document medical conditions, interventions, and changes in health status.
Communicate medical concerns and recommendations to the Director of Nursing and appropriate program leadership.
Provide health-related training and in-service education to residential staff.
Assist participants with medical appointments and transportation when required.
Ensure nursing practices and documentation comply with applicable regulations and program standards.
Participate in residential team meetings and collaborate with interdisciplinary teams.
Advocate for participants’ health, safety, dignity, rights, and overall quality of life.
Maintain confidentiality and promote respectful, person-centered care.
Perform other duties as assigned.

Associate's degree required
Standard driver’s license – class C or class D
Registered Nursing License required (RN)
3 – 5 years of related experience
2 years supervisory experience.
