Job Overview
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The LPN will provide high-quality nursing care to patients across various settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. This role involves performing patient assessments, administering medications and treatments, and supporting the overall health and well-being of diverse patient populations. Duties
- Conduct comprehensive patient assessments, including vital signs, physical examinations, and health histories.
- Administer medications, injections—including IV infusions—and treatments following physician orders and safety protocols.
- Assist with diagnostic evaluations such as specimen collection, processing, and medical imaging procedures.
- Monitor patients’ conditions closely, recognizing signs of acute pain or deterioration, and escalate concerns appropriately.
- Maintain accurate medical documentation within electronic health record (EHR) systems while ensuring compliance with HIPAA regulations.
- Support discharge planning by providing patient education on medication management, care procedures, and follow-up requirements.
- Manage catheter care, wound dressing changes, and other specialized nursing interventions.
- Collaborate effectively with interdisciplinary teams including physicians, therapists, caregivers, and families to coordinate comprehensive care plans.
- Ensure infection control practices are followed rigorously to maintain a sterile environment.
- Participate in ongoing training related to emergency medicine, trauma response, dementia care—including Alzheimer’s and memory care—and other specialized areas.
Skills
- Valid licensure as a Licensed Practical Nurse (LPN) with current certification in CPR/BLS.
- Experience working with diverse patient populations across settings such as long-term care facilities, hospitals—including ICU and trauma centers—and outpatient clinics.
- Proficiency in EMR/EHR systems like Epic, Cerner, or Athenahealth for accurate documentation.
- Strong knowledge of medical terminology, physiology, anatomy, and pharmacology relevant to nursing practice.
- Skilled in medication administration techniques including injections and IV insertions; experience with IV infusion therapy is highly desirable.
- Familiarity with ICD coding (ICD-9/ICD-10), CPT coding for procedures, and documentation standards for compliance.
- Ability to perform physical examinations and vital sign assessments accurately.
- Knowledge of infection control protocols and aseptic techniques to prevent healthcare-associated infections.
- Excellent caregiver communication skills with the ability to educate patients about their health conditions effectively.
- Capable of working under pressure in fast-paced environments such as emergency rooms or urgent care centers while maintaining attention to detail.
- Experience working with individuals with disabilities or complex health needs such as dementia or behavioral health conditions is preferred.
