Acuity Home Health is a growing Medicare-certified home health agency committed to delivering exceptional skilled nursing and therapy services throughout the Chicagoland area. We're looking for a motivated Registered Nurse who enjoys both patient care and relationship building.
This unique hybrid position combines clinical nursing with healthcare business development, allowing you to care for patients while helping expand access to quality home healthcare throughout the community.
What You'll Do
Clinical Nursing Responsibilities
- Provide skilled nursing care in patients' homes according to physician orders and individualized care plans.
- Perform assessments, treatments, medication management, wound care, and patient education.
- Accurately document visits in WellSky Kinnser EMR.
- Collaborate with physicians, therapists, and the interdisciplinary care team.
- Promote patient safety and quality outcomes.
Business Development Responsibilities
- Visit physician offices, hospitals, skilled nursing facilities, rehabilitation centers, and assisted living communities.
- Build and maintain relationships with physicians, discharge planners, case managers, social workers, and office staff.
- Educate referral partners about Acuity Home Health's services.
- Conduct follow-up phone calls, emails, and in-person meetings.
- Track marketing activities and referral development.
- Represent Acuity Home Health professionally within the healthcare community.
Qualifications
- Active Illinois RN license in good standing.
- Valid driver's license and reliable transportation.
- Excellent communication and relationship-building skills.
- Self-motivated, organized, and comfortable working independently.
- Home health and OASIS experience preferred but not required.
- Experience with WellSky Kinnser is a plus.
- CPR certification.
