What You’ll Do
- Develop and manage individualized hospice care plans for your patients.
- Monitor and manage pain, symptoms, and overall comfort.
- Serve as the primary RN and trusted guide for patients and families.
- Navigate complex situations with calm, confident clinical judgment.
- Coordinate closely with physicians, social workers, chaplains, and aides.
- Provide care in homes, assisted living, and skilled nursing environments.
- Maintain accurate, timely documentation to support quality outcomes.
What We’re Looking For
- Active Minnesota Registered Nurse license.
- Experience in hospice, oncology, or chronic illness management.
- Strong communication, organization, and problem-solving skills.
- Ability to work independently while staying connected to the team.
- Deep commitment to dignity, autonomy, and patient-centered care.
