Legacy Homecare Solutions is seeking compassionate, dependable, and professional Home Care Aides (HCAs) to provide personal care services for adolescent children to senior adults with intellectual disabilities in the comfort of their homes. We are looking for Home Care Aides who are committed to delivering exceptional service while promoting dignity, independence, safety, and quality of life.
We are currently seeking qualified Home Care Aides for part-time opportunities providing care throughout Alameda County, Contra Costa County, and the East Bay. This position offers consistent 12-hour shifts with flexible scheduling. Available schedules include Friday and Saturday day shifts (7:00 AM – 7:00 PM) and overnight shifts (7:00 PM – 7:00 AM).
Responsibilities
As a Home Care Aide, you may assist clients with:
- Personal care assistance (bathing, grooming, dressing, and hygiene)
- Toileting and incontinence care
- Mobility assistance and mobility support using equipment like Hoyer lifts when necessary for safe transfers
- Medication reminders
- Meal planning and preparation
- Light housekeeping and laundry
- Companionship and emotional support
- Transportation to appointments and community activities
- Safety supervision and fall prevention
- Monitoring and reporting changes in client condition
- Accurate completion of daily care notes and documentation
- Monitor client's by observing changes in condition, and reporting findings accurately while adhering to HIPAA regulations to ensure privacy and confidentiality
- Maintain a clean and safe environment by performing light cleaning duties, laundry, and ensuring hygiene standards are met within the home or care facility.
- Provide companionship and emotional support to foster social engagement and reduce feelings of isolation for clients with dementia or Alzheimer’s disease.
- Collaborate with nursing staff, social workers, and other healthcare professionals to implement individualized care plans focused on patient safety and well-being.
- Use patient observation skills to identify potential issues early, including signs of distress or deterioration related to conditions such as dementia, autism spectrum disorder, or developmental disabilities.
- Support hospice or palliative care needs by providing respectful comfort measures aligned with patient wishes.
Experience
- High School Diploma or GED preferred.
- Minimum one (1) year of professional caregiving, home care, direct support, or personal care experience preferred.
- Experience providing assistance with Activities of Daily Living (ADLs), including bathing, grooming, dressing, toileting, transferring, and ambulation.
- Experience supporting individuals with intellectual and developmental disabilities (IDD), including Cerebral Palsy, Down Syndrome, Autism Spectrum Disorder, Intellectual Disabilities, and other developmental conditions.
- Experience providing Supported Living Services (SLS), Independent Living Services (ILS), respite care, personal assistance services, or home care services preferred.
- Experience promoting independence, self-determination, and community integration for individuals with disabilities.
- Experience implementing individualized care plans, support plans, and client-centered goals.
- Experience providing companionship, emotional support, and behavioral support in a professional manner.
- Experience supporting clients with mobility limitations, physical disabilities, and adaptive equipment.
- Experience documenting daily activities, client progress, incidents, and observations accurately and timely.
- Experience communicating effectively with clients, families, case managers, supervisors, and interdisciplinary teams.
- Experience providing transportation assistance and accompanying clients to appointments, community activities, and recreational outings.
- Experience working independently in a client's home while maintaining professionalism, confidentiality, and safety standards.
- Experience recognizing and reporting changes in a client's physical, emotional, or behavioral condition.
- Experience working with seniors, individuals with dementia, Alzheimer's disease, cognitive impairments, or developmental disabilities is highly preferred.
- Experience working with diverse populations such as seniors or individuals requiring long-term or hospice care is advantageous.
- Certification in CPR and First Aid is required
- Ability to perform physical tasks including heavy lifting safely while maintaining compassion and professionalism.
- Demonstrated knowledge of caring for individuals with developmental disabilities, autism spectrum disorder, dementia (including Alzheimer’s disease), or memory care needs
