What You'll Do
As an RN Case Manager, you'll oversee and coordinate the care of hospice patients while serving as a vital member of the interdisciplinary care team.
Key Responsibilities
- Manage and coordinate individualized patient care plans
- Conduct comprehensive nursing assessments and ongoing evaluations
- Monitor patient conditions and respond to changing needs
- Administer medications and treatments as ordered
- Educate patients and families regarding disease progression, symptom management, and care needs
- Collaborate closely with physicians, social workers, chaplains, aides, and other care team members
- Ensure accurate and timely clinical documentation
- Advocate for patient comfort, dignity, and quality of life
- Provide compassionate support to patients and families throughout their hospice journey
QualificationsRequired
- Active Registered Nurse (RN) license in good standing
- Current CPR certification
- Valid driver's license and reliable transportation
- Strong clinical assessment, communication, and organizational skills
Preferred
- Hospice, home health, palliative care, oncology, geriatric, or acute care nursing experience
- Case management experience
