Duties
- Conduct thorough patient assessments, including vital signs, symptom management, and physiological evaluations to inform individualized care plans.
- Administer medications and treatments in accordance with physician orders and hospice protocols, ensuring patient safety and comfort.
- Collaborate closely with interdisciplinary teams—including physicians, social workers, chaplains, and therapists—to coordinate holistic patient care.
- Manage electronic health records (EHR/EHR) systems accurately, documenting all patient interactions, assessments, and interventions in compliance with HIPAA regulations.
- Provide education and emotional support to patients and their families about disease progression, symptom control, and end-of-life options.
- Participate in discharge planning, ensuring seamless transitions between inpatient, outpatient, home health, and post-acute care settings.
- Engage in case management activities such as patient service coordination, utilization management, and diagnostic evaluation to optimize quality of life.
Skills
- Proven critical care experience with a strong background in hospital medicine, ICU, emergency medicine or trauma center environments.
- Experience working within Level I or Level II trauma centers or similar high-acuity settings; familiarity with infectious disease care is a plus.
- Knowledge of medical terminology, physiology, and health regulation policies to deliver precise and compliant care.
- Skilled in managing complex patient cases involving geriatrics, behavioral health issues, pediatrics or post-acute care needs.
- Proficient in EMR/EHR systems for documentation and communication; understanding of HIPAA standards for patient privacy.
- Excellent patient service skills with the ability to communicate effectively with diverse populations and collaborate with healthcare professionals.
- Capable of performing comprehensive patient assessments including vital signs and clinical case management activities across inpatient and outpatient environments.

Pay: $43.27 - $44.47 per hour
