We are currently seeking a Case Manager, CMA or LPN ,to join our team within our Central Business Office located in Oldsmar FL.
The position offers the possibility to becoming remote once the training and introductory period has been completed.
The case manager interacts with patients to perform initial interviews and screenings to determine patient needs, assists in scheduling patient appointments and assists patients with resources as needed.
Key Responsibilities:
- Conduct telephonic outreach to patients identified with recent hospitalizations, identified as high risk and high cost, having frequent emergency room visits or chronic conditions.
- Screening and assessment of patients to promote successful transitions of care, management of chronic conditions, and need for resources and education. Uses clinical judgement to assess and evaluate patients at risk for repeat hospitalization and ER visits.
- Provide clear communication with the patient and provider. Communication of patient status and plan of care with provider as needed.
- Documentation of all communication and care provided in patient’s EMR.
Minimum Qualifications;
Education– HS Diploma minimum
Job Experience– One (1) year of pediatric experience in a clinical setting.
- Knowledgeable in Case Management principles, procedures, and practices.
- Knowledgeable in clinical quality indicators (HEDIS, Care Gaps).
Professional License/Certification– Current CMA, RMA or LPN.
