Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong clinical background, with experience in various care settings including long-term, acute, and rehabilitative care. As an LPN, you will be responsible for delivering direct patient care, assisting with medical procedures, and ensuring the safety and well-being of our residents and patients through attentive assessment and compassionate support.
Responsibilities
- Provide routine nursing care to residents and patients in accordance with physician orders and established protocols.
- Monitor vital signs, administer medications, and perform injections while adhering to safety standards such as HIPAA and aseptic techniques.
- Assist with complex procedures including IV infusion, catheterization, wound suturing, and tube feeding.
- Conduct patient assessments, document medical records accurately using Point Click Care, and report changes in condition promptly.
- Support discharge planning, coordinate with interdisciplinary teams for post-acute care, and participate in case management activities.
- Manage infection control protocols and ensure compliance with healthcare regulations across various settings including hospice, geriatrics, pediatrics, and behavioral health.
- Collaborate effectively with healthcare professionals in hospital medicine, emergency rooms and other providers.
Qualifications
- Valid LPN license in the state of practice with current certification.
- Proven experience in diverse healthcare environments such as hospitals (including trauma centers), nursing homes, assisted living facilities, or home & community care settings.
- Familiarity with medical terminology, ICD coding (ICD-9/10), CPT coding, and medical documentation standards.
- Experience administering injections, phlebotomy, venipuncture, sterile processing techniques, and specimen collection/processing.
- Knowledge of physiology, anatomy, pharmacology related to pain management, dementia care (including Alzheimer’s and memory care), and infectious disease protocols.
- Strong communication skills with the ability to work effectively with people of diverse backgrounds including those with developmental disabilities or requiring specialized dementia or hospice care.
