Job Responsibilities:
Assess, plan, implement, and evaluate patient-centered care plans that address social, emotional, and psychological needs of patients
Develop and maintain strong relationships with patients, families, and healthcare teams to ensure comprehensive care
Provide access to individual, group, and family therapy sessions to address mental health concerns, coping strategies, and life skills
Conductevidence-based interventions to address patient-specific needs, including crisis intervention, case management, and community resource coordination
Collaborate with healthcare providers to develop patient-centered care plans and improve healthcare outcomes
Maintain accurate and timely documentation of patient services and progress
Requirements:
Current licensure as a Licensed Social Worker (LSW) in the state of Minnesota
Minimum of 3 years of experience in social work, preferably in a long-term healthcare setting
Strong communication, interpersonal, and organizational skills
Ability to work effectively in a fast-paced, dynamic environment
