CLUES' Case Manager provides culturally responsive, bilingual Targeted Case Management services to both children with Serious Mental Illness (SMI) and adults with Serious and Persistent Mental Illness (SPMI) who reside in Ramsey County and meet Minnesota eligibility criteria. The Case Manager works collaboratively with clients, families, community partners, and providers to coordinate services, improve access to behavioral health care, and promote recovery, stability, and community integration.
The Case Manager develops individualized service plans, coordinates community resources, provides ongoing support, responds to crises, and ensures compliance with county, state, and agency requirements.
CLUES' Certified Community Behavioral Health Clinic (CCBHC), Abriendo Caminos, provides immediate access to supportive, high-quality, culturally responsive, and integrated behavioral health services regardless of insurance status or ability to pay. The CCBHC model supports coordinated, person-centered care and expands access to behavioral health services for the Latinx community.

Estimated Pay: $22.00/hr to $26.00/hr (depending on experience & qualifications).

Provide bilingual, culturally responsive Targeted Case Management services to eligible children and adults in accordance with Minnesota statutes and Ramsey County requirements.
- Build professional relationships with clients and families to assess needs, identify strengths, and facilitate access to behavioral health, medical, educational, vocational, social, housing, and other community resources.
- Complete required assessments and service planning documents in a timely manner, including Functional Assessment, Individual Family Community Support Plan (IFCPS) or Individual Community Support Plan (ICSP) as applicable, and develop client case plans and crisis plans.
- Review and update these plans every 180 days.
- Maintain accurate, timely, and complete documentation, including case notes and required agency reporting.
- Coordinate services among providers and monitor implementation of client treatment and support plans.
- Meet with clients at least monthly or more frequently as clinically indicated and required by program and county standards for client contracts.
- Respond appropriately to client crises and collaborate with supervisors and other professionals during high-risk situations.
- Support clients during mental health appointments, school meetings, medical appointments, court hearings, commitment proceedings, and other service-related activities as appropriate.
- Provide transportation when appropriate and in accordance with agency policy.
- Support adults involved in civil commitment processes and assist with discharge planning and community reintegration when applicable.
- Collaborate with Behavioral Health leadership and multidisciplinary teams to provide integrated, person-centered care.
- Participate in individual supervision, group supervision, staff meetings, training, and other required agency activities.
- Develop and maintain collaborative relationships with county representatives, schools, healthcare providers, social service agencies, and community partners.
- Promote the mission, values, and services of CLUES within the community.
- Maintain client confidentiality in accordance with HIPAA, agency policies, and all applicable laws and ethical standards.
- Perform other duties as assigned.

Candidates must meet the qualifications for a Mental Health Case Manager under applicable Minnesota Statutes and Minnesota Health Care Programs (MHCP) requirements.
Candidates must meet one of the following qualifications:

Be qualified as a Mental Health Practitioner under Minnesota Statutes.
- Hold at least a bachelor's degree in a behavioral science or related field (including, but not limited to, Social Work, Psychology, or Nursing) from an accredited college or university.
- Meet one of the alternative qualification pathways for Mental Health Case Managers established under Minnesota Statutes.

Experience providing case management or direct behavioral health services preferred.
- Knowledge of community resources serving children, adults, and families preferred.
- Experience working with individuals experiencing serious mental illness and/or severe emotional disturbance preferred.
- Skilled in assessing client needs and coordinating services.
- Crisis intervention and de-escalation experience preferred.
- Strong organizational, documentation, and time management skills.
- Ability to manage multiple priorities in a fast-paced environment.
- Demonstrated cultural responsiveness and knowledge of Latinx cultures, values, and traditions.
- Effective communication in English and Spanish, both verbally and in writing, required.

Ability to read, analyze, and interpret written materials, including reports, contracts, and proposals.
- Ability to effectively present ideas and information in and organized and effective manner, including writing reports.
- Ability to maintain accurate documentation and respond appropriately to questions from clients, co-workers, management and external parties
- Must have access to use a reliable motor vehicle for independent transportation, valid driver’s license, proof of automotive insurance, and maintain a good driving record. Must submit to and pass annual recertification of compliance with CLUES Automobile and Transportation Policy and driving record check.
- Proficient working knowledge of email software, the internet, databases, and common Windows-based programs, including Microsoft Office, Word, Excel and PowerPoint required.
