We are located at 3230 Pennsylvania Ave, SE, Washington DC 20020, and we serve patients in both DC and MD. With our unique care model, we aim to grow rapidly both regionally and nationally.
Essential job duties and responsibilities
We are looking for an LPN to join our team. You will work closely with our other clinical staff members, under supervision of a nurse practitioner, physician assistant or doctor. The responsibilities will be, amongst others:
- Monitor patient’s condition and assess their needs to provide the best possible care and advice
- Observe and interpret patient’s symptoms and communicate them to our providers
- Collaborate with providers to create and execute individualized care plans for patients
- Perform routine procedures (blood pressure measurements, administering injections, cleaning and dressing of wounds, point-of-care tests etc.) and accurately document your findings
- Care management
- Remote patient monitoring
- Adjust and administer patient’s medication and provide treatments according to practitioners' orders
- Provide emotional and psychological support when needed
- Supervise and train LPNs and nursing assistants
- Assist with providing care at partner facilities
- Foster a supportive and compassionate environment to care for patients and their families
- Expand knowledge and capabilities by leading educational workshops, conferences etc.
- Communicate with patients’ family or friends to provide advice, comfort and release instructions
- Utilize in-office, in-home, and virtual visits to maximize impact on patients' health
- Help the team be a high performing team, delivering a unique patient experience
- Perform accurate and timely documentation in the electronic medical record.
- Perform other related duties as assigned.
This position is a full-time position and is fully in-person.
Qualifications

The ideal candidate has a strong affinity or connection with the populations we serve. Key criteria include:
- Minimum of 1 year work experience as LPN, RN, or similar role in in outpatient setting (preferably primary care)
- Ability to undertake a variety of diverse care tasks and work in a multidisciplinary team
- Knowledge of primary care specific safety and sanitation standards and procedures
- Understanding of confidentiality obligations and nursing best practices
- Excellent knowledge of medical terminology
- Excellent communication and interpersonal skills
- Willingness to learn, collaborate in care teams
- Compassionate and able to handle stress
- Successful completion of LPN program is a must
- Strong computer skills, including experience with Electronic Medical Records and office software.

Pay: $50,000.00 - $75,000.00 per year
