American Traveler is seeking an experienced Physical Therapist for a Home Health position covering Snohomish and Skagit County, requiring a minimum of 1 year of recent experience and proficiency with HCHB and OASIS documentation.

Home health setting serving a geriatric patient population across Snohomish and Skagit County
-
Territory coverage of up to a 60-mile radius from the hub/office; travelers are expected to be flexible with territory

Caseload of approximately 5–7 patients per day with a minimum of 6 visits daily

Minimum productivity expectation of 30 points per week

Schedule is Monday–Friday, 8-hour day shifts (8:00 AM–5:00 PM) with no weekend or on-call requirements

Clinician is expected to visit the main office at least once a week or every two weeks for team meetings, supply pickup, and admission packets

EMR: HCHB (experience required)

OASIS experience required

Minimum 1 year of recent Physical Therapy experience in home health

Active PT license required if applicable

Current BLS certification required

Proficiency with HCHB EMR required

OASIS documentation experience required

Must be able to travel within a 60-mile service territory

Strong time management skills with the ability to meet productivity and documentation standards

First-time travelers will not be considered for this position

Responsibilities include conducting home visits, completing timely documentation, and maintaining consistent communication with the care team

An iPad is provided for use in the field; personal cell phone is not required

Mileage is reimbursed at the IRS standard rate

Scrubs in any color are required; jeans are not permitted

Local candidates are accepted and eligible for the travel pay rate
