Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using tools like Hoyer lifts and catheter care when needed
- Monitor and record vital signs such as blood pressure, pulse, temperature, and respiration to ensure patient health and safety
- Administer medications accurately following medical documentation protocols while maintaining compliance with HIPAA regulations
- Provide specialized care for clients with dementia, Alzheimer’s disease, or other memory impairments through tailored memory care techniques
- Support clients with medical procedures such as wound care, hospice support, and working with EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems for documentation
- Prepare nutritious meals and assist with meal planning and cooking to promote healthy living in accordance with dietary needs
- Maintain a clean and safe environment by assisting with light housekeeping tasks including laundry and general cleaning
Skills
- Strong patient observation skills to detect changes in condition or behavior promptly
- Experience working in assisted living facilities, nursing homes, or home health environments with long-term care experience
- Knowledge of medical terminology relevant to caregiving tasks and patient records management
- Proficiency in using EMR/EHR systems for accurate medical documentation and record keeping
- Ability to work effectively with individuals who have developmental disabilities or require dementia and Alzheimer’s care
- Certification in CPR (Cardiopulmonary Resuscitation) and training in hospice care practices preferred
- Familiarity with medical equipment such as Hoyer lifts, vital signs monitors, catheters, and medication administration protocols
- Compassionate communication skills to build trust and rapport with clients and their families
- Flexibility to work live-in shifts or part-time schedules as needed in home & community care settings
