WHAT YOU’LL DO
The Registered Nurse (RN) Case Manager is responsible for coordinating and managing patient care. The RN Case Manager assesses patient needs, develops individualized care plans, coordinates services among healthcare providers, educates patients and families, and ensures quality, cost-effective care while promoting positive patient outcomes.
POSITION REQUIREMENTS
· Active, unrestricted California RN license — REQUIRED
· Minimum 1 of years of direct home health agency experience (must be reflected on resume) REQUIRED
· Minimum 1 of years of direct supervisory roles
· Current CPR certification — REQUIRED
· Reliable transportation and valid driver's license — REQUIRED
· May require 4 days of on-call availability per month, with weekend and holiday on-call rotations— REQUIRED
· Language: Russian — REQUIRED
