Schedule and Location
Employment type: Full-time (open to part-time for the right candidate)
Schedule: Monday–Friday, 8:00 AM – 5:00 PM (flexibility for intake coverage)
Work setting: On-site — Dallas–Fort Worth, TX [Plano] with Hybrid-remote options
About the role
The Intake Assessor is a licensed behavioral health clinician who conducts comprehensive clinical intake and admission assessments for patients entering our intensive outpatient program (IOP) in Dallas–Fort Worth. You’ll determine clinical appropriateness and level of care, formulate initial diagnostic impressions, and ensure each patient begins treatment with an accurate, trauma-informed clinical picture. This role bridges access and clinical care — partnering with an interdisciplinary treatment team to deliver a safe, seamless admission.
What You’ll Do
- Conduct comprehensive biopsychosocial assessments for prospective patients
- Determine clinical appropriateness and recommend the appropriate level of care
- Conduct clinical risk assessment, including suicidal/homicidal ideation, withdrawal risk, and acute safety concerns
- Develop preliminary treatment recommendations and initial treatment plans in collaboration with the clinical team
- Communicate clinical determinations and handoffs clearly to treating clinicians and program staff
- Facilitate warm handoffs and referrals to external providers or alternative levels of care when a patient is not appropriate for admission
- Serve as the welcoming first point of contact for incoming patient calls and referrals, responding in a timely, professional, trauma-informed way
- Schedule, confirm, and reschedule assessments and appointments within established timeframes
- Manage intake and referral tracking so no inquiry or referral is ever lost or delayed
- Conduct follow-up outreach for missed calls, no-shows, and pending assessments
- Keep accurate, up-to-date records across all intake and tracking systems
What we’re looking for
- Texas behavioral health license — LCSW, LPC, LMSW, or LPC-Associate (LPC-A).
- Master’s degree in social work, counseling, psychology, or a related behavioral health field
- Experience in behavioral health, mental health, or substance use treatment
- Strong organization and multitasking; comfortable managing high-volume workflows with attention to detail
- Skilled in biopsychosocial assessment, DSM-5-TR diagnosis, and clinical risk assessment
- Familiarity with insurance authorization and utilization management processes
- Proficiency with EHR systems (e.g., AdvancedMD, Netsmart) and strong clinical documentation skills
- Excellent clinical interviewing, communication, and decision-making abilities
- Prior experience conducting intake/admission assessments and level-of-care determinations strongly preferred
- Experience in IOP, PHP, or outpatient settings preferred
Why Ascent Healthcare Texas
At Ascent Healthcare Texas, we build systems that lift people up. We seek out what’s broken — not to criticize it, but to restore it — and we work relentlessly to make things better for our patients, our teams, and our future. We believe trauma disrupts choice, chaos disrupts clarity, and shame disrupts connection, so we build the opposite: structure that creates safety, clarity that creates confidence, accountability that creates empowerment, and collaboration that creates stability. We don’t separate compassion from standards — we integrate both, because from the first phone call to discharge, every touchpoint matters. If you want to do meaningful clinical work on a team that restores agency, holds high standards, works across disciplines, and improves relentlessly, we’d love to meet you. Here, high standards aren’t about perfection — they’re about consistency. And healing is never rushed; it’s steady, upward, measured, and sustainable.
