We are seeking a dedicated Registered Nurse to join our Hospice team, where you’ll play a vital role in providing skilled, patient-centered care to through end-of-life care with dignity, comfort, and compassion.
Key Responsibilities

Provide skilled nursing care to hospice patients in their homes using the full nursing process.

Assess the physical, mental, emotional, and social needs of patients and families.

Develop, implement, and update individualized plans of care in coordination with the interdisciplinary hospice team.

Administer medications and treatments as prescribed and provide education on disease progression, symptom management, medications, and comfort care.

Serve as case manager for assigned patients, coordinating care with physicians, caregivers, team members, and community resources.

Support patients and families in coping with illness, loss, and end-of-life decisions.

Accurately and timely document assessments, interventions, and care provided in the electronic medical record.

Participate in interdisciplinary team meetings, quality improvement efforts, and the hospice on-call rotation.
Qualifications

Current Texas RN license or compact equivalent.

BLS for Healthcare Providers certification required.

Valid driver’s license, current auto insurance, and reliable transportation required.

Minimum of one year of RN experience preferred.

Prior hospice experience preferred.

Strong communication, organizational, interpersonal, and critical thinking skills.

Ability to provide compassionate care and emotional support to patients and families in a home-based care setting.
