Pay: $53,500 – $65,216 per year
As a Counselor on our Assertive Community Treatment (ACT) Team you will provide evidence-based, multidisciplinary programing and provide comprehensive, coordinated care to adults with serious and persistent mental illness and co-occurring substance use disorders. This role delivers face-to-face screenings, assessments, crisis intervention, and individual/group counseling across diverse community settings in a trauma-informed environment.
Key Responsibilities
- Assessment & Care Planning: Conduct intake interviews, evaluate mental/emotional distress and substance use, and develop individual service plans.
- Clinical Services: Provide short- and long-term individual and group psychotherapy, addressing both mental health and co-occurring substance abuse.
- Crisis & Community Support: Perform face-to-face screenings and crisis interventions; connect clients with appropriate community resources.
- Team Collaboration: Coordinate care across internal programs and external agencies; provide clinical support to TBS Specialists/Managers as needed.
- Documentation & Compliance: Maintain complete case notes and records according to ACT fidelity guidelines and productivity expectations.
- Outreach: Provide consultation, education, and educational talks to community groups.
Role Requirements
- Education & Licensing: Master’s degree in a mental health–related field with active LSW or LPC licensure required (LISW, LPCC, MFT, or IMFT preferred).
- Experience: Background in mental health/substance abuse assessment, crisis intervention, and therapy preferred; EHR experience a plus.
- Transportation: Valid Ohio driver’s license, clean driving record, reliable personal vehicle, and proof of adequate auto insurance required for community travel.
