Skilled Respite Nurse (LVN or RN) – Multiple Cases Available
We are seeking a compassionate and skilled nurse to provide one-on-one care in the home setting while allowing primary caregivers essential respite time. Position Overview
The Skilled Respite Nurse provides direct clinical care to medically complex patients in their homes under the supervision of a Registered Nurse and/or physician. Nurses in this role support patient stability, safety, and comfort while maintaining open communication with the interdisciplinary care team.
Pay & Schedule Transparency
- LVN Pay Range: $26.00 – $31.00 per hour
- RN Pay Range: $32.00 – $38.00 per hour
(Final rate based on clinical experience, case complexity, and shift commitment)
- Shift Lengths: Typically 4-hour, 6-hour, or 8-hour shifts
- Availability: Day, evening, overnight, weekday, and weekend cases available
- Consistency: Many cases offer set recurring schedules for continuity of care
- Environment: One patient per shift — no facility ratios
Reliable attendance and schedule commitment are essential due to the nature of respite services.
Key Responsibilities
- Provide skilled nursing care to patients in the home to support caregiver relief and continuity of care
- Administer medications, treatments, and enteral feedings as ordered
- Monitor and document vital signs, clinical status, and patient response to interventions
- Recognize and promptly report changes in patient condition
- Assist with ongoing patient assessments and contribute to individualized care planning
- Perform accurate and timely documentation in electronic health record systems
- Educate patients and family members on disease processes, safety measures, and care techniques
- Maintain strict adherence to infection control, safety standards, and agency policies
Clinical Skills & Experience
- PEG tube care and enteral feedings
- Safe medication administration
- Aspiration precautions and airway safety monitoring
- Ventilator and/or tracheostomy care
- Foley catheter care and monitoring
- Skin integrity management and pressure injury prevention
- Experience caring for medically complex patients in home health, pediatric, or long-term care settings preferred
Qualifications
- Current Texas LVN or RN license in good standing
- Basic Life Support (BLS) certification required
- Strong understanding of nursing standards of practice
- Knowledge of medical terminology and anatomy
- Experience with electronic documentation systems preferred
- Ability to work independently in a home environment while collaborating with the care team
