Access Ohio Behavioral Health, an outpatient community behavioral health agency is hiring part-time/full-time Registered Nurses (RN's) / Licensed Practical Nurse (LPN), for our offices located in Delaware & Mt. Gilead. Our offices are open Monday - Friday between 8am-6pm (no weekends required). This is an office/community split position with a 70% productivity standard and requires some services to be provided to clients in their homes and other community settings. Pay based upon experience and licensure (LPN/RN).
Experience in mental health and substance abuse disorders preferred.
Job Responsibilities:
- Diagnose and establish patient treatment plans/complete intake health assessments
- Monitor and report changes in patient symptoms or behavior
- Communicate with interdisciplinary team i.e Prescribers, Case Managers and Counselors regarding patient care and compliance with treatment.
- Educate patients about treatment and medications
- Facilitate referrals to other healthcare professionals and medical facilities
- Maintain accurate patient medical records
- Community/Home visits as needed
Skills to have:
- Proven experience as a registered nurse or licensed practical nurse
- Excellent knowledge of nursing care methods and procedures
- Excellent knowledge of emergency care
- In-depth knowledge of health and safety guidelines and procedures (sanitation, decontamination etc.)and willingness to follow them at all times
- A team player with excellent communication and interpersonal skills
- Responsible and compassionate
- Outstanding organizational and multi-tasking skills
- Patient with excellent problem-solving skills
- BS or other diploma from a nursing program
Requirements:
- Valid Nursing License in Ohio (LPN or RN)
- Valid Ohio Driver’s License with no more than 2 points
- Reliable transportation
- Current automobile liability insurance
- Clean urine drug screen upon hire (zero tolerance policy)
- Background check upon hire
- Obtain and approved Ohio Department of Medicaid provider agreement and updated to include agency-level association within 3 days of hire.
Pay: $28.00 - $39.00 per hour
