Reporting to the Day Habilitation Asst. Director, the Day Hab program support coordinator responsibilities include supervision of assigned Day Habilitation Disabilities Specialist including performance evaluations. Ensure staff action plans, goals and all other plans are implemented with the individuals and that all staff are trained in these plans. Responsible for checking precision care to ensure all required meetings have been held, all timelines have been met and all required documents have been uploaded to Pcare. Ensure all OPWDD/Medicaid requirements/standards for meetings and billing are met.
Participate in required meetings, including medical, clinical, Day Program, and Life Plan meetings; ensure recommendations are implemented.
Oversee coordination and follow-up of participants’ needs and goals through ongoing observation in program and community settings, maintaining direct contact to understand behavioral, social, and functional needs.
Maintain a schedule for semi-annual and annual planning meetings with care managers, team members, families, and other supports. Ensure meeting minutes reflect progress, supports, and updates.
Ensure staff and individuals are trained and actively implementing goal-related and outcome-based activities across settings, with appropriate supervision and individualized support.
Review, correct, and approve staff action plans, including goals, outcomes, and community inclusion activities. Certify plans on a semi-annual and annual basis or as needed.
Ensure each individual has a current Life Plan and related documentation on file and uploaded to Pcare. Ensure case records are maintained quarterly.
Serve as liaison with families, guardians, and external providers as needed.
Review and certify monthly summaries, ensuring timely completion and accurate reflection of progress.
Review behavior plans, medical information, and staff action plans to determine if updates or meetings are needed.
Ensure timely and accurate collection of service data, including behavioral tracking and community inclusion schedules.
Complete all reports and documentation on time.
Ensure individuals are safe, clean, and supported in an appropriate environment. Ensure proper supervision levels and staffing coverage.
Attend intake and admissions meetings; develop initial staff action plans and ensure ongoing updates with input from all stakeholders.
Report incidents immediately and assist with required documentation.
Advocate for program participants.
Provide and participate in staff training and professional development, maintaining required certifications.
Provide coverage for other supervisors as assigned and complete additional tasks as needed.
