We are currently seeking Licensed Practical Nurses (LPNs) who are passionate about making a difference and growing their careers in a collaborative healthcare environment.
About the Role
The Licensed Practical Nurse (LPN) plays a vital role in delivering safe, efficient, and patient-centered care across Community Health Net locations. This position includes a $3,000 sign-on bonus, payable in accordance with company policy and contingent upon meeting all employment requirements.
LPNs may be required to float between CHN locations based on operational needs.
Key Responsibilities
- Conduct and accurately document patient health histories and assessments

Assist medical providers in delivering high-quality, patient-centered care

Manage patient flow, including triage, follow-up care, and care coordination

Coordinate referrals to specialists and community-based services

Provide health education to patients and caregivers

Collect and process laboratory specimens (e.g., blood, urine, sputum)

Support infection control practices, safety standards, and appropriate use of supplies

Assist with front office functions as needed (scheduling, patient discharge, etc.)

Float between Community Health Net locations as required.
Qualifications
- Active Pennsylvania Licensed Practical Nurse (LPN) license

Current CPR certification

High school diploma or GED
Required Skills & Competencies
- Strong clinical assessment and organizational skills

Excellent verbal and written communication abilities

Knowledge of medical terminology and basic insurance types

Proficiency with computers and electronic health record (EHR) systems

Ability to work collaboratively in a fast-paced healthcare environment
