Job Summary
We are seeking a dynamic and experienced Nurse Supervisor for Start of Care and Supervisor Visits to lead our home health nursing team with energy, precision, and compassion. In this pivotal role, you will oversee the initiation of patient care plans, coordinate supervisor visits, and ensure compliance with healthcare regulations. Duties
- Meet with client and staff during Start of Care assessments and supervisor visits, ensuring adherence to clinical protocols and regulatory standards.
- Coordinate patient intake processes, including comprehensive assessments, vital signs collection, and medical documentation.
- Develops initial personalized care plans for diverse patient populations, IDD, geriatric, pediatric, adult, and behavioral health.
- Ensure compliance with HIPPA
- Monitor clinical quality metrics such as vital signs accuracy, medication administration compliance, catheter care standards, and tube feeding protocols to uphold high standards of patient safety.
Requirements
- Valid RN license with current state licensure
- Experience with Homecare Assessments
- Proficiency with Microsoft office products
- Strong knowledge of medical terminology, anatomy, physiology, ICD-10/ICD-9 coding systems, and diagnostic evaluation techniques.
- Proven supervisory experience managing nursing teams within home health or Skilled Nursing Facility
- Familiarity with healthcare regulations including HIPAA
- Skills in patient assessment—vital signs monitoring, physical examinations.
- Ability to handle complex cases involving tracheostomy management, IDD, and behavioral health.
- Excellent communication skills
