We are seeking a Michigan-licensed medical professional to complete and sign DHS-54A Medical Needs forms for clients enrolled in the Michigan Home Help Program.
This is a fully remote position with flexible scheduling. The provider will review client information and certify medical necessity for Home Help services in accordance with Michigan Medicaid guidelines.
All documentation is submitted electronically.
Responsibilities:
- Review client medical information
- Complete and sign DHS-54A Medical Needs forms
- Enter NPI and Medicaid enrollment status on required documentation
- Ensure compliance with Michigan Medicaid and CHAMPS requirements
- Maintain accurate and timely documentation
Required Qualifications (Must Be Michigan Licensed):
Applicant must hold an active, unrestricted Michigan license in one of the following:
- Physician (MD or DO)
- Physician Assistant (PA-C)
- Nurse Practitioner (NP)
- Occupational Therapist (OT)
- Physical Therapist (PT)
✔ Must have active NPI
✔ Must be enrolled or eligible to enroll as a Michigan Medicaid provider
✔ Must be able to work remotely
Compensation:
- Per-form payment structure (flat rate per DHS-54A completed)
