PRN Occupational Therapist (OT) – Home Health
Location: Deerfield Beach, Hillsboro Beach, Pompano Beach, Lauderdale-By-The-Sea, Fort Lauderdale, Dania Beach, and Hallandale Beach.
Position Summary:
We are seeking a compassionate and self-motivated Occupational Therapist (OT) to join our Home Health team on a per diem (PRN) basis. In this role, you will provide individualized, in-home therapy services to help patients regain independence in their daily living and self-care activities. Responsibilities:
- Perform initial and ongoing assessments of patients’ functional abilities, ADLs, mobility, safety, and home environment.
- Develop and implement individualized care plans based on physician orders and patient needs.
- Deliver therapeutic interventions including training in self-care, use of adaptive equipment, and home safety modifications.
- Educate patients and caregivers to support recovery and independence.
- Accurately document all care provided using WellSky.
- Coordinate care and communicate effectively with the interdisciplinary home health team.
- Maintain compliance with agency, state, and federal guidelines.
- Current FL licensure as an Occupational Therapist (OT).
- Minimum 1 year of clinical experience; home health experience preferred.
- Familiarity with WellSky EMR or similar documentation systems preferred.
- Strong clinical reasoning, time management, and communication skills.
- Ability to work independently and manage a flexible field-based schedule.
- Reliable transportation, valid driver’s license, and current auto insurance.
