Care Coordinator – Clinical Services
Full-Time | Monday–Friday Day Shift
Wages: $30.50 - $35.00/hour DOE

We are seeking a dependable, patient-focused Care Coordinator to play a critical role in supporting our providers, coordinating patient care, and ensuring an exceptional clinic experience.
This is a hands-on clinical support role that requires strong follow-through, attention to detail, and the ability to manage multiple moving pieces throughout the patient care process.
What You’ll Do
- Room patients and obtain/document vital signs
- Perform medication reconciliation and update patient records
- Coordinate clinical follow-up and patient care plans
- Track patients who have been seen but are not yet scheduled for procedures
- Assist providers with thyroid biopsies and other office-based procedures
- Support phlebotomy and basic lab workflows including orders, requisitions, specimen collection, labeling, and tracking
- Review provider notes and coordinate imaging, labs, and follow-up requirements
- Communicate with patients regarding next steps, scheduling, and care coordination
- Help ensure smooth patient flow and operational efficiency throughout the clinic day

The most important characteristic for success is strong organization and reliable follow-through on patient tracking and care coordination. We are looking for someone who is proactive, detail-oriented, and committed to ensuring that no patient “falls through the cracks.”
Qualifications
- Graduate of a Practical Nursing program
- LPN licensure required
- BLS/CPR certified or willing to obtain certification
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced clinical environment
- Minimum 1 year of clinical experience preferred
