Job Summary
We are seeking a dedicated and caring Licensed Practical Nurse (LPN) to provide exceptional care to our residents in a supportive and team-oriented environment. You will play an essential role in delivering quality nursing care, promoting dignity, and helping residents achieve their highest level of well-being.
Essential Duties and Responsibilities:
- Resident Care
- Provide direct nursing care to assigned residents following individualized care plans and physicians’ orders.
- Administer medications and treatments accurately and timely in accordance with facility policies and state regulations.
- Observe residents and report significant changes in condition to the RN, physician, or appropriate supervisor.
- Assist with admissions, transfers, and discharges of residents.
- Perform and document ongoing assessments, interventions, and outcomes in the electronic health record (EHR).
- Supervision and Leadership
- Supervise, guide, and support nursing assistants to ensure quality resident care and adherence to care plans.
- Promote teamwork and open communication among staff to maintain a positive, respectful work environment.
- Participate in staff education, training, and performance feedback as assigned.
- Documentation and Communication
- Accurately chart all medications, treatments, and nursing interventions.
- Communicate effectively with residents, families, physicians, and interdisciplinary team members.
- Participate in resident care conferences and quality improvement activities.
- Safety and Compliance
- Follow infection control, safety, and emergency procedures at all times.
- Maintain confidentiality of resident information in accordance with HIPAA regulations.
- Ensure all nursing practices comply with state and federal long-term care requirements.
Qualifications:
- Current Licensed Practical Nurse (LPN) license in the State of Minnesota.
- Current CPR/BLS certification.
- Prior experience in long-term care or geriatric nursing preferred, (but not required—new grads welcome!).
- Ability to communicate effectively and compassionately with residents, families, and staff.
- Strong attention to detail, organization, and professionalism.
- Ability to lift, move, and assist residents as required.
