Duties
- Conduct thorough psychosocial assessments and diagnostic evaluations to determine patients’ emotional, social, and behavioral needs.
- Develop and implement individualized care plans that incorporate community resources, discharge planning, and behavioral health interventions.
- Provide individual, family, and group counseling sessions utilizing techniques such as motivational interviewing, cognitive-behavioral therapy, and behavioral therapy.
- Coordinate with interdisciplinary teams to facilitate patient care transitions across inpatient, outpatient, hospice, or post-acute settings.
- Manage medical documentation accurately within electronic medical records (EMR) systems while adhering to HIPAA privacy regulations and social work practice standards.
- Offer crisis intervention and mental health counseling for patients experiencing acute emotional distress or behavioral health crises.
- Support specialized populations including children & families, seniors with dementia or Alzheimer's disease, individuals with developmental disabilities or autism, and those requiring end-of-life or PTSD care.
Skills
- Proven experience in hospital settings or hospice & palliative medicine with knowledge of medical terminology and clinical ethical guidelines.
- Expertise in child & family counseling, addiction counseling, crisis management, and working with individuals with disabilities or developmental challenges.
- Strong case management skills including intake procedures, discharge planning, data collection, and medical records documentation.
- Familiarity with EMR systems, CPT coding, ICD-9/10 coding standards for accurate clinical documentation.
- Ability to perform diagnostic evaluations using tools like ICD coding systems; experience with Medicare requirements is a plus.
- Exceptional communication skills for conducting individual/group therapy sessions and collaborating effectively within multidisciplinary teams.
- Knowledge of community resource coordination to connect patients with appropriate services such as assisted living or senior care programs.
