Position Summary
The Hospice RN Case Manager is responsible for coordinating and managing the care of an assigned caseload of hospice patients while serving as the primary clinical contact for patients, families, physicians, and the interdisciplinary team.
This role provides comprehensive nursing assessments, develops individualized care plans, manages symptoms, educates families, and advocates for patients throughout their hospice journey.
As a Case Manager, you'll have the opportunity to build meaningful relationships with patients and families while delivering the highest level of compassionate end-of-life care.
Primary Responsibilities
As an RN Case Manager, you will:
- Complete comprehensive patient and family assessments upon admission and throughout hospice care.
- Develop, implement, and revise individualized plans of care.
- Manage pain and symptoms while promoting comfort and dignity.
- Coordinate care with physicians, nurse practitioners, social workers, chaplains, hospice aides, volunteers, and other interdisciplinary team members.
- Educate patients and families regarding disease progression, medications, symptom management, and end-of-life care.
- Administer medications and treatments as ordered.
- Communicate patient condition changes promptly with physicians and the care team.
- Maintain timely, accurate, and compliant clinical documentation.
- Supervise Hospice STNAs and delegate patient care appropriately.
- Coordinate equipment, medications, and community resources necessary for patient care.
- Participate in interdisciplinary team meetings and care conferences.
- Rotate participation in the on-call schedule.
- Serve as an advocate for patients and families while honoring their goals and wishes.
Qualifications
Qualified candidates will have:
- Current Ohio Registered Nurse (RN) license.
- Graduate of an accredited nursing program (BSN preferred).
- Current CPR certification.
- Minimum one year of nursing experience required.
- Home Health and/or Hospice experience preferred.
- Excellent assessment, communication, documentation, and critical thinking skills.
- Compassion for patients and families facing end-of-life care.
- Valid driver's license, reliable transportation, and current automobile insurance.
- Ability to work independently while collaborating effectively within an interdisciplinary team.
