Position Summary
The Registered Nurse (RN) is responsible for conducting initial client assessments, reassessments, supervisory visits, care plan reviews, and quality assurance monitoring for Sharinglyfe Care LLC. The RN ensures that all clients receive appropriate non-medical home care services according to agency policies, state regulations, and client needs.
This position does not provide routine skilled nursing services but serves as the clinical oversight professional responsible for evaluating client needs, monitoring caregiver performance, and ensuring quality care delivery.
Essential Duties and ResponsibilitiesClient Assessments
- Complete initial in-home assessments for all new clients.
- Evaluate client physical, cognitive, emotional, and social needs.
- Assess Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs).
- Determine appropriate caregiver staffing and service recommendations.
- Develop and review individualized service plans.
- Document all assessment findings accurately and timely.
Reassessments
- Conduct scheduled reassessments every 6 months or as required by agency policy.
- Perform reassessments whenever there is a significant change in a client's condition.
- Update service plans and care needs accordingly.
- Communicate changes to agency management, caregivers, and responsible parties.
Supervisory Visits
- Conduct supervisory visits in client homes.
- Observe caregiver performance and compliance with the care plan.
- Ensure caregivers are providing services safely and appropriately.
- Review documentation, timesheets, and client satisfaction.
- Provide coaching, education, and corrective guidance to caregivers as needed.
- Complete supervisory visit reports according to agency requirements.
Care Plan Management
- Create and update client service plans.
- Ensure services remain appropriate for the client's needs.
- Recommend adjustments to care schedules and caregiver assignments.
- Coordinate with family members, physicians, case managers, and other healthcare professionals when appropriate.
Quality Assurance
- Monitor quality of care provided by direct care workers.
- Investigate complaints, incidents, and client concerns.
- Participate in agency quality improvement activities.
- Assist with compliance reviews and audits.
- Ensure agency records meet state and agency requirements.
Staff Support & Training
- Assist in caregiver orientation and training.
- Provide education regarding infection control, safety, dementia care, fall prevention, and client rights.
- Support supervisors with caregiver performance concerns.
- Participate in staff meetings and in-service training.
Documentation
- Complete assessment reports.
- Complete supervisory visit forms.
- Complete reassessment forms.
- Maintain accurate client records.
- Ensure all documentation is completed within required timeframes.
Qualifications
- Current Mississippi Registered Nurse (RN) License in good standing.
- Minimum of one (1) year nursing experience preferred.
- Home care, home health, hospice, long-term care, or case management experience preferred.
- Strong assessment and documentation skills.
- Excellent communication and interpersonal skills.
- Ability to work independently.
- Valid driver's license and reliable transportation.

Pay: $32.00 - $55.00 per hour
