Preferred Medical Group is actively recruiting a full-time Chronic Care and Remote Patient Monitoring Coordinator.
Preferred Medical Group is a multispecialty medical practice with locations in Phenix City, Fort Mitchell, Opelika, and Auburn, Alabama. This position supports eligible patients with Medicare plans through our Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs.
The coordinator will help patients manage their chronic conditions through regular communication, care coordination, remote monitoring, preventive care, and wellness support. Strong communication, attention to detail, compassion, and follow-through are essential. The relationships we build with our patients play an important role in helping them achieve better health outcomes.
Responsibilities:
- Identify and enroll eligible patients in CCM and RPM programs.
- Explain program requirements, obtain consent, and educate patients about participation.
- Provide ongoing outreach and support in accordance with CMS guidelines.
- Review RPM readings and alerts daily.
- Contact patients regarding missing, abnormal, or concerning readings.
- Promptly communicate abnormal readings and patient concerns to the appropriate clinical staff.
- Assist patients with basic RPM device setup and troubleshooting.
- Coordinate appointments, referrals, testing, medication concerns, and other healthcare needs.
- Follow up on outstanding orders and help ensure care coordination items are completed.
- Develop and maintain accurate, patient-specific care plans.
- Document patient communication, monitoring activities, care coordination, and time spent in the electronic health record.
- Track monthly CCM and RPM requirements to support accurate billing and CMS compliance.
- Work proactively and with urgency when addressing time-sensitive patient needs.
- Protect patient privacy and follow HIPAA and organizational policies.
- Collaborate with providers, medical assistants, and other members of the care team.
Qualifications:
- Minimum of one year of Chronic Care Management experience required.
- Remote Patient Monitoring or other care coordination experience preferred.
- Experience using an electronic health record system preferred.
- Ability to learn quickly and work productively in a fast-paced environment.
- Strong organizational, time-management, and follow-up skills.
- Ability to prioritize and manage multiple responsibilities.
- Excellent written and verbal communication skills.
- Ability to work effectively both independently and as part of a team.
- Compassionate, professional, and patient-focused approach.
- Spanish-speaking skills are preferred but not required.
Job Type: Full-time
Pay: $16.00 - $18.00 per hour
