Licensed Practical Nurse (LPN) – Assisted Living Facility
Location: Rye, New York
Schedule: Mix of Evenings and Nights
We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join a respected Assisted Living Facility in Rye, New York. This is a great opportunity for an LPN who is looking for flexible evening shifts while providing high-quality care in a supportive, resident-centered environment.
The LPN will be responsible for providing direct nursing care, administering medications, monitoring residents’ health, and supporting the daily clinical needs of the facility.
Responsibilities:
Provide nursing care to residents in accordance with care plans and facility policies
Administer medications and treatments as prescribed
Monitor residents for changes in condition and report concerns promptly
Assist with resident assessments and documentation
Communicate professionally with residents, families, and the care team
Support CNAs and other care staff as needed
Maintain a safe, respectful, and comfortable environment for residents
Requirements:
Active LPN license in the state of New York
Assisted Living, Long-Term Care, or Skilled Nursing experience preferred
Strong communication and documentation skills
Reliable, compassionate, and team-oriented
Availability to work evening and night shifts (3:30pm to 11:30pm and 11:30pm to 7 :30am)
