Part Time Certified Social Worker Wanted at an Adult Day Care Center
Certified Social Worker wanted at a state of the art adult medical day care center located in Fort Lee. New to the field? Just graduated? No worries, we are willing to train!
Responsibilities include, but are not limited to:
Interview participants and participants' caregiver(s) in order to assess and evaluate needs and concerns.
Provide or arrange for individual, family and group counseling in reference to psychological, social, financial, legal, vocational and educational needs of the participant.
Assist with obtaining concrete services, such as housing, shopping, and more.
Refer to or develop support groups and educational programs for the participants and their caregivers.
Arrange or provide crisis intervention.
Provide family outreach.
Coordinate participants' care plans with other in-house or community resources.
Provide in-service training to staff on participant and caregiver psychosocial needs.
Participate in the day care facility’s quality improvement program.
Participate in professional organizations and seminars.
Participate in participant’s case conferences, such as pre-admissions, post admissions or problem-oriented matters.
Document assessments, treatment plans, evaluations and clinical notes.
Coordinate discharge plans and link participants to necessary community services.
In addition, the social worker is expected to contribute toward maintaining a friendly, warm, collegial and inviting environment for all concerned. Must comply with company policies and procedures and take in-house or outside training programs as required. Expected to treat participants with respect, professionalism, care and interest, patience and understanding. Must be a team collaborator with excellent communication skills.
Pay commensurate with experience and educational background. While not required, experience in an adult day care setting is a plus. Please send resume along with 2-3 references and a copy of your current NJ CSW certificate.
Pay: $27.00 - $30.00 per hour
