Job Summary
We are seeking a compassionate and dedicated Hospice Nurse to join our team. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring comfort and dignity during end-of-life care. This role requires a strong commitment to patient-centered care, effective communication skills, and the ability to work collaboratively with a multidisciplinary team.
Duties
- Assess patient needs and develop individualized care plans in collaboration with the healthcare team.
- Administer medications and IV infusions as prescribed, ensuring adherence to safety standards.
- Provide nursing care in various settings, including home community care and ambulatory surgery centers.
- Monitor patients for changes in condition and respond appropriately, including providing dementia care when necessary.
- Educate patients and families on disease processes, treatment options, and end-of-life planning.
- Maintain accurate and timely documentation using electronic health records systems such as WellSky.
- Adhere to established protocols and standards of care within a Level II Trauma Center environment.
- Collaborate with physicians, social workers, and other healthcare professionals to ensure comprehensive patient support.
Skills
- Valid nursing license with experience in hospice or palliative care preferred.
- Proficient in IV infusion techniques and knowledge of anatomy related to patient care.
- Strong understanding of ER protocols and standards for emergency situations.
- Excellent interpersonal skills with the ability to communicate effectively with patients, families, and team members.
- Ability to provide empathetic support while managing complex emotional situations related to end-of-life care.
- Strong organizational skills with attention to detail in documentation and patient assessments.
