Job Overview
We are seeking a dynamic and compassionate Family Nurse Practitioner (FNP) to join our dedicated healthcare team. Responsibilities
- Conduct detailed physical examinations and health assessments for patients of all ages, including pediatrics, geriatrics, and special populations such as assisted living residents and individuals with developmental disabilities.
- Develop and implement individualized care plans, including medication management, health education, and preventive strategies.
- Perform clinical procedures such as suturing, IV insertion, catheterization, and dermal filler administration with precision and adherence to sterile techniques.
- Utilize electronic health record (EHR) systems like Epic, eClinicalWorks, or Cerner to document patient encounters accurately and efficiently.
- Manage acute and chronic illnesses by ordering diagnostic tests, interpreting results—including laboratory tests like ICD-10 coding—and coordinating follow-up care.
- Provide telehealth consultations to expand access to care while maintaining high standards of patient engagement and safety.
- Collaborate with multidisciplinary teams across specialties such as gastroenterology, infectious disease care, behavioral health, hospice & palliative medicine, and senior care to ensure comprehensive patient management.
Requirements
- Valid Family Nurse Practitioner license with current certification from the American Association of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC).
- Proven experience in primary care settings with proficiency in medical documentation, CPT coding, ICD coding (ICD-9/10), and utilization review processes.
- Hands-on experience with medical procedures including suturing, phlebotomy, venipuncture, sterile processing techniques, and injection administration.
- Familiarity with EMR/EHR systems such as Epic, Cerner, or Athenahealth for efficient patient record management.
- Strong knowledge of physiology, anatomy, pharmacology (including psychopharmacology), and diagnostic evaluation methods.
- Experience working in diverse environments such as hospital medicine units—including ICU or Level I/II trauma centers—urgent care clinics, outpatient clinics, nursing homes or assisted living facilities.
- Ability to perform triage assessments quickly and accurately while managing acute pain or airway emergencies when necessary.
- Excellent communication skills with a passion for patient education on topics like family planning, nutrition, memory care (including Alzheimer’s/dementia), and post-discharge planning. Pay: $110,000.00 - $120,000.00 per year
