Overview
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care and support under the supervision of registered nurses and physicians. This role is essential in delivering effective nursing services, ensuring patient comfort, and maintaining a safe environment for all patients.
Responsibilities
Provide direct patient care, including monitoring vital signs, administering medications, and performing basic nursing procedures.
Assist with patient assessments and report changes in condition to the RN or physician.
Conduct in-home supervisory visits(45-60 days) to evaluate the quality of care provided by caregivers or aides.
Perform wound care, catheterization, injections, and other clinical tasks as directed.
Maintain accurate and timely patient records in compliance with agency and regulatory requirements.
Educate patients and families on care plans, medication use, and health maintenance.
Assist with activities of daily living (ADLs) for patients as needed.
Ensure infection control and safety protocols are followed at all times.
Collaborate with the healthcare team to promote positive patient outcomes
Qualifications
Valid Licensed Practical Nurse (LPN) license in the state of practice.
Experience in emergency medicine or medical office settings is preferred.
Proficiency in medication administration and specimen collection processing.
Strong organizational skills and attention to detail.
Excellent communication skills, both verbal and written.
Ability to work effectively within a team-oriented environment.
Previous experience with electronic health records (EHR) systems such as Cerner or Athenahealth is a plus.
Knowledge of utilization management principles is beneficial.
