Job Title: Director of Social Services

- College degree required.
- Licensed Social Worker may be required based on regulations and facility size.
- Skilled Nursing experience preferred.
Responsibilities:
- Plans, develops, organizes, implements, evaluates, and directs the social service programs of the facility.
- Participates in discharge planning, development, and implementation of social care plans and resident assessments.
- Interviews residents and families as necessary to identify medically related social and emotional needs.
