Schedule: Monday–Friday, 8:00 AM – 5:00 PM (with occasional on-call responsibilities)
The Hospice Clinical Manager is responsible for overseeing patient care services within a hospice/home health program. This role functions as part of the nursing leadership team and serves as a key communication link between clinical staff, support teams, referral sources, and leadership.
The Clinical Manager provides operational oversight, staff mentorship, scheduling coordination, and performance management, while ensuring the highest standards of patient care.
- Oversee daily hospice patient care services and clinical operations
- Collaborate closely with the Director of Clinical Services
- Provide onboarding, training, mentoring, and ongoing support to clinical staff
- Participate in patient assessments and care planning
- Assist clinicians with crisis intervention and problem-solving
- Participate in on-call rotation for Clinical Managers
- Assume operational oversight in the absence of senior leadership
- Monitor staff performance and conduct evaluations
- Identify performance gaps and implement corrective action plans
- Participate in recruitment, interviewing, and hiring processes
- Ensure appropriate staffing levels and schedule coordination
- Approve timesheets and manage leave requests
- Coordinate with HR on employee safety and workplace incidents
- Promote and ensure safe patient care practices
- Review incident reports and initiate corrective actions within 24 hours
- Address patient or family concerns in a timely manner
- Oversee timely patient admissions and scheduling
- Collaborate with intake and billing teams
- Monitor supply usage and coordinate with vendors
- Conduct utilization and productivity analysis
- Provide regular reports to leadership
- Monitor budget adherence and investigate variances
- Track and improve key performance indicators
- Organize and lead staff meetings
- Maintain documentation and follow-up on action items
- Act as a liaison between departments and field teams
- Registered Nurse (RN)
- Active, unrestricted RN license in Montana
- Minimum 3 years of hospice experience
- Strong analytical, organizational, and interpersonal skills
- Excellent verbal and written communication skills
- Proficiency in Microsoft Office and digital platforms
- Bachelor of Science in Nursing (BSN)
- Prior supervisory or leadership experience
