Perform all pharmacist functions including compounding, dispensing, verification, and documentation of medications across central, satellite, and IV areas.
Review and monitor medication orders and patient profiles to prevent, identify, and resolve drug-related problems; complete pharmacist consults and perform pharmacokinetic dosing, monitoring, and adjustments.
Provide safe, timely, and cost-effective medication therapy; actively participate in stewardship, dose optimization, surveillance, and restricted-use programs.
Engage in direct patient care and interdisciplinary rounds, ensuring smooth transitions of care from admission through discharge and follow-up.
Educate and counsel patients, families, and healthcare team members on safe and effective medication use.
Supervise, mentor, and evaluate pharmacy technicians, interns, students, and residents; support team learning and development.
Participate in quality improvement initiatives, regulatory compliance, and accreditation activities; track performance metrics (e.g., interventions, error reduction, cost savings).
Identify, report, and help resolve medication safety events, adverse drug reactions, and errors.
Strong verbal and written communication, organizational, problem-solving, and customer service skills
Flexibility and willingness to adapt to changes as necessary
Works under minimal supervision, while using independent judgment
Ability to interact constructively with supervisors, peers, and support personnel
Ability to analyze data and follow up and resolve discrepancies

Ability to participate in clinical activities and documentation such as IV to PO, renal dosing, and pharmacokinetics
Initiative to keep abreast of developments in the provision of pharmacy services
Initiative to keep abreast of new information/products/developments in the field of pharmacy
Proficiency in general hospital pharmacy operations and regulatory compliance
Proficiency with hospital computer systems, pharmacy technology, and pharmacy automation
Proficiency in hospital pharmacy practice, patient-centered care, advanced pharmacy practice, medication therapy management, and evidence-based medicine
Must be able to read, write and speak conversational English
Basic computer skills and knowledge of Microsoft Office
Performs other duties as assigned
Fully understands and can apply all state and federal laws and regulations related to the practice of pharmacy
Must possess excellent knowledge of current drug therapies, drug interactions, pharmacokinetics, and sociologic principles necessary to assist in the safe and effective utilization of drugs by patients
Demonstrate knowledge and proficiency regarding special needs and behaviors of age or ethnic specific groups
Demonstrated ability to follow written and verbal instructions effectively
Knowledge of drugs and their indications, contraindications, dosing, side effects, and proper administration
Knowledge of patient care charts and patient histories, with the ability to perform basic patient assessments and referrals
Ability to maintain quality, safety, and/or infection control standards
Knowledge of related accreditation and certification requirements
Ability to communicate technical information to non-technical personnel
Proficiency in utilizing electronic health records (EHR) and clinical decision support systems
Perform Clinical Consultations on Glycemic Management, Anticoagulation, and Pharmacokinetics
Must possess the ability to establish and maintain effective relation
American Society of Health-System Pharmacist (ASHP) Board Certification [Preferred]
