Location: Primary Care and Family Practice Clinic in Davenport, WA (Serving Davenport, Reardan and Wilbur Clinics)
Salary Range: $45-65/hour (DOE)
Schedule: Full-Time | 40 hours/week

Lincoln Hospital District #3 is seeking a visionary RN Clinical Specialist to help design, launch, and lead an innovative care coordination program across our Family Practice Clinics. This is not a traditional case management position — it’s an opportunity for a motivated RN to build something new, drive quality, and directly influence how we deliver care for our rural community. You’ll use your clinical expertise and critical thinking to create workflows, educate staff, and implement programs that transform patient care — with the autonomy to make decisions and the support of a leadership team that values forward-thinking ideas.

What You’ll Do
- Partner with providers to design and implement patient-centered care plans
- Lead initiatives in chronic disease management (diabetes, hypertension, COPD, etc.)
- Build and manage disease registries that support proactive outreach and measurable improvement
- Coach and train staff (MAs, LPNs, etc.) to elevate team-based care delivery
- Develop and participate in group visits, wellness programs, and new clinical initiatives
- Use data to drive quality improvement and care innovation
What We’re Looking For
- Graduate of an accredited school of nursing; Current WA RN license
- 3–5 years of nursing experience (primary care, chronic disease, care coordination, or leadership preferred)
- Strong clinical judgment with the ability to think independently and strategically
- Proven leadership, teaching, and coaching skills
- EMR proficiency and understanding of clinic quality metrics
- Preferred: Certified Diabetes Educator (or willingness to obtain)
