Schedule: Full-Time Days | Monday–Friday | Occasional weekends as needed
Compensation: $31.25–$46.88/hour (Salary-Based Position)

- Assess patient needs and develop individualized discharge plans
- Collaborate with providers, nursing staff, patients, families, and community resources
- Facilitate transitions of care and connect patients with appropriate post-acute services
- Monitor utilization and ensure compliance with regulatory and payer requirements
- Promote quality outcomes, patient safety, and efficient resource management
