POSITION SUMMARY:
The Registered Nurse demonstrates critical thinking skills, scientific judgment, and leadership in planning for the care of the patient. The Registered Nurse is responsible and accountable for the planning, directing, and provision of nursing care to patients in accordance with the NH Nurse Practice Act, recognized nursing techniques, and both clinical and administrative policies.
SPECIFIC ELEMENTS & ESSENTIAL FUNCTIONS:
1. Completes position responsibilities by following established guidelines and protocols within the appropriate time frame.
2. Demonstrates good skills in all forms of communication. Works well with others in a spirit of teamwork and cooperation.
3. Maintains education and development appropriate for the position.
4. Adheres to established hospital and department guidelines and protocols. Supports hospital mission, vision, and values.
5. Performs position specific job functions at an acceptable level.
6. Demonstrates sensitivity and respect to the family and/or significant other as an individual and family unit.
7. Identifies cultural and religious needs, language barriers, developmental needs, etc., and modifies plan of care as necessary to accommodate those needs.
8. Demonstrates understanding of end-of-life decisions and provides appropriate support for Advanced Directives, and DNR decisions.
9. Serves as an advocate for patients, family, and significant other(s) in regard to decisions affecting the plan of care.
10. Verbalizes the process for referral when ethical dilemmas arise.
11. Provides for the privacy of patients/family.
12. Performs a thorough assessment in accordance with unit standards and/or patient condition.
13. Based on assessment, plans, and implements care appropriately, including the patient/family/significant other, whenever possible.
14. Reassesses patient in accordance with unit standards and/or patient condition.
15. Utilizes ancillary services appropriately.
16. Assesses and monitors patient for seclusion and restraint in accordance with hospital policy.
17. Recognizes and reports signs and symptoms of abuse and neglect.
18. Provides nursing care within scope of practice and according to policy, for patient population of assigned unit(s).
19. Develops and implements a comprehensive plan of care based on initial assessments involving the patient and/or family.
20. Anticipates changes in acuity, needs/problems and expectations through continuing evaluation of patient data; implements care as appropriate.
21. Demonstrates calm and efficient demeanor while intervening during emergency situations.
22. Performs environmental rounds; reports faulty equipment/damage and safety issues to appropriate person.
23. Assesses patient educational needs and provides appropriate follow-up for educational needs.
24. Implements discharge plan in timely manner, in collaboration with other health team members.
25. Successfully assumes resource nurse responsibility. Designates work assignment considering individual staff abilities and patient needs, including changes in acuity level. Coordinates unit activities. Anticipates staffing requirements.
26. Adheres to unit/hospital standards for documenting patient care.
27. Completes annual competencies within appropriate time frame.
28. Performs all other duties as needed.
