Job Overview
We are seeking a dedicated and compassionate Social Services Assistant to support residents' emotional, social, and psychological well-being. The ideal candidate will possess strong interpersonal skills, a thorough understanding of social work principles, and experience working with diverse populations including geriatric patients, individuals with disabilities, and those requiring behavioral health services. This paid position offers an opportunity to make a meaningful difference in residents' lives through case management, crisis intervention, and supportive counseling.
This position involves collaborating with insurance companies, hospitals, healthcare providers, patients, and their families to facilitate communication, obtain eligibility and authorizations, and coordinate case management for the center. The ideal candidate will possess a strong understanding of managed care, insurance authorization, and skilled documentation for nursing and therapy services.
Responsibilities
- The Social Services Assistant will assist the Social Service Director in the planning, developing, organizing, implementing, evaluating, and directing of social services programs of this facility.
- The Social Services Assistant will meet with administration, medical and nursing staff, and other related departments in planning social services, as directed.
- The Social Service Assistant will participate in discharge planning, development and implementation of care plans and resident assessments.
- The Social Services Assistant will encourage residents/responsible parties to participate in the development of their care plan, and invite them to care plan meetings accordingly.
- The Social Services Assistant will assist residents in obtaining transportation to medical appointments, upon discharge, etc.
- The Social Services Assistant will accurately and completely document social service actions and interactions in each resident’s medical record.
- The Social Services Assistant will engage in Advance Care Planning for assigned residents upon admission, and make sure that any Advance Directives are reviewed with the resident/resident representative on a regular basis. The Social Worker will ensure that staff members are made aware of the resident’s code status and end-of-life wishes and will assist with informing and educating residents and their representatives about health care options and ramifications.
- The Social Services Assistant will ensure that residents who display mental illness, or psychosocial difficulties such as coping with grief and loss, have access to appropriate treatment and resources.
- Serve as the primary point of contact for patients, addressing inquiries and concerns related to their skilled care stay.
- Obtain insurance authorizations to facilitate patient admission.
- Deliver NOMNC and ABN notifications.
- Maintain professional communication with insurance representatives, patients, peers.
- Coordinate patient appointments, ensuring timely access to services and follow-up care.
- Set meetings with patient responsible parties to coordinate plan of care and set discharge goals.
- Collaborate with healthcare providers to streamline communication and enhance patient care.
- Educate patients on treatment options, procedures, and necessary follow-up actions.
- Ensure compliance with healthcare regulations and maintain confidentiality of patient information.
- Coordinate service for discharge planning including DME, home health, and other support services.
Additional Assigned Tasks
Contributes to the facility efforts to maintain and/or improve quality of care through participation in the following:
- Attends Care Plan meetings.
- Serves as a member of the QAPI Committee as requested.
- Serves as a member of the Behavior Management Committee.
- Attends Department Head/Standup Meetings.
- Attends mandatory in-services.

Qualifications
- Proficiency in medical terminology is essential for effective communication within the healthcare environment.
- Strong understanding of Medicare Part A and Managed Care / Medicare Advantage as it relates to services in a skilled nursing environment.
- Previous experience in a skilled nursing facility is highly preferred with experience in Case Management, LPN or RN, Social Services, Admissions, Therapy, or similar roles.
- Strong organizational skills with the ability to manage multiple tasks simultaneously while maintaining attention to detail.
- Excellent interpersonal skills to foster positive relationships with patients, families, and healthcare professionals.
- Ability to work collaboratively within a team-oriented, fast-paced environment while demonstrating initiative in problem-solving.
- Must maintain excellent attendance.
