Responsibilities
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record
- Administer medications and injections accurately following prescribed protocols while adhering to HIPAA regulations and safety standards.
- Assist with medical procedures such as catheterization, tube feeding, wound healing, and sterile processing techniques.
- Monitor patients’ conditions closely, manage acute pain, and respond promptly to emergencies.
- Collaborate with interdisciplinary teams on discharge planning, case management, and coordination of post-acute or home & community care services.
- Conduct specimen collection/processing, perform phlebotomy and venipuncture procedures, and maintain meticulous medical documentation.
- Support specialized care needs including dementia care, Alzheimer’s care, hospice care, infectious disease management, and working with individuals with developmental disabilities or disabilities.
Experience
- Proven experience as an LPN in diverse healthcare environments such as hospitals, nursing homes, outpatient clinics, or home care settings.
- Demonstrated proficiency with EMR/EHR systems like Epic or Point Click Care for accurate medical records management.
- Experience working with adult or geriatric populations.

Pay: $22.00 - $24.00 per hour
