Wellsential Health is seeking an experienced and clinically skilled RN, LVN or RRT to serve as a Clinical Nurse Liaison supporting the admissions and care evaluation process for our skilled nursing facility. This role is ideal for a nurse with a strong background in long-term care, MDS, ADON, nurse management or similiar Clinical Liaison experience who understands the level of care our facility can safely provide, and can confidently screen, assess, and transition patients from hospital to SNF.
What You’ll Do:
- Review hospital referrals and clinical notes to determine admission appropriateness
- Conduct in-person clinical assessments at area hospitals to evaluate care needs
- Collaborate with DON, Administrator, and Admissions Director to ensure appropriate placement
- Communicate clearly with case managers, families, and interdisciplinary teams
- Prevent “failed admits” by matching resident needs to facility capabilities
- Coordinate with therapy, dietary, and nursing to ensure continuity of care
What We’re Looking For:
- LVN, RN or RRT license (required)
- Minimum 2 years in long-term care or skilled nursing
- Experience as an MDS nurse, ADON, or Nurse Manager preferred
- Solid understanding of Medicare/Medicaid coverage and hospital discharge processes
- Strong clinical judgment and communication skills
- Ability to read between the lines in referral packets and advocate for realistic care planning
- Reliable transportation and flexibility to conduct hospital visits
